Jeremiah Heath Rockman was born and died on Monday, Dec. 29, 2014, in Milwaukee. Jeremiah was the son of Cierra Bernarde and Derrick Rockman.
Jeremiah is survived by his mom, Cierra; his dad, Derrick; a brother, Avery Perez; grandmothers, Melissa Heath and Theresa Escalante; grandfather, Marc Bernarde; great-grandmother, Leah Miller; and numerous aunts and uncles.
Funeral services will be held at 1:30 p.m. Saturday, Jan. 3, 2015, at the Church of the Wilderness with the Rev. Melinda Shriner officiating. Visitation will be at the church on Saturday from noon until the time of the service.
Swedberg Funeral Home is assisting the family with the arrangements.
